Caregivers and providers can be challenged with providing care and activities for their parents, short-term and long-term. Join us in our state-of-the-art facility during the day and experience our exceptional service.

Most insurance is accepted, and we also provide monthly and weekly financial terms.

Eligibility for specific services is determined by a case manager during an in-home assessment.

Funding Sources

PASSPORT is a Medicaid program administered through the Council on Aging of Southwestern Ohio (COA). The purpose of the program is to allow seniors 60 years of age and older to receive nursing facility level of care while remaining in their home.

For PASSPORT eligibility please contact the Council on Aging of Southwestern Ohio for an assessment with a case manager at 513-721-1025.

Frequently Asked Questions

Adult day service centers provide a coordinated program of professional and compassionate services for adults in a community-based group setting. Services are designed to provide social and some health services to adults who need supervised care in a safe place outside the home during the day.

Comprehensive, therapeutic activities are structured daily to support an individual’s health and independence goals. Supervision and hands on assistance with personal hygiene activities  are provided as needed.  Required health assessments and monitoring of health status will be performed.

Transportation service is provided within a limited geographic area.  Routes are planned so that participants are on the vehicle one hour or less. Our transportation schedule is designed to be as consistent and efficient as possible. Pick up and drop off times are based on your location.

Lunch and snacks are provided daily. We are able to meet most dietary needs such as: low/no sugar, texture modification, low cholesterol, etc.

Monday through Friday 7:00 am to 5:00 pm

Adult day service and transportation services are completely free for Medicaid clients under Medicaid’s long-term care / HCBS program, unless shared payments are applicable.

We accept Medicaid (PASSPORT, MyCare Ohio (Aetna, Molina, Care Star, Care Source), Elderly Services Program, and Veterans Administration.
