JOB TITLE:   LHOI Adult Day Service Program Manager

REPORTS TO:     Executive Director

Position Description

The Adult Day Service Manager is responsible for overseeing the daily operations of the adult day service program, ensuring the delivery of high-quality care and services to participants. This role involves managing staff, coordinating activities, and maintaining compliance with all relevant regulations and standards.

Key Responsibilities 

  • Program Management:
    • Develop and implement daily schedules and activities that meet the physical, emotional, and social needs of participants.
    • Ensure the program provides a safe, clean, and stimulating environment.
    • Monitor and evaluate program effectiveness, making adjustments as needed.
  • Staff Supervision:
    • Recruit, train, and supervise program staff and volunteers.
    • Conduct regular performance evaluations and provide ongoing support and development opportunities.
    • Ensure staff compliance with all policies, procedures, and regulatory requirements.
  • Participant Care:
    • Conduct assessments of prospective participants to determine eligibility and appropriate care plans.
    • Coordinate with caregivers, families, and healthcare professionals to develop and implement individualized care plans.
    • Monitor participant progress and adjust care plans as necessary.
  • Compliance and Documentation:
    • Maintain accurate and up-to-date records for all participants and staff.
    • Ensure compliance with all state and federal regulations, including health and safety standards.
    • Prepare and submit required reports to regulatory agencies and funding sources.
  • Financial Management:
    • Manage the program budget, including billing, reimbursements, and expense tracking.
    • Identify and pursue funding opportunities to support program growth and sustainability.
  • Community Engagement:
    • Develop and maintain relationships with community organizations, healthcare providers, and other stakeholders.
    • Promote the program through outreach and marketing efforts to increase awareness and participation.

Qualifications 

  • Bachelor’s degree in social work, healthcare or business administration, or a related field (Master’s preferred).
  • Minimum of 3 years of experience in a supervisory role within an adult day service or similar setting.
  • Strong knowledge of state and federal regulations related to adult day services.
  • Excellent organizational, communication, and leadership skills.
  • Ability to work effectively with diverse populations and manage multiple priorities.

 Physical Demands:

  • Ability to lift and carry up to 25 pounds.
  • Frequent bending, stooping, and walking.
  • Ability to respond to emergency situations as needed.

 Work Environment:

  • Primarily indoor work in a day service facility.
  • Occasional travel for meetings, training, and community outreach.

Activity Coordinator

JOB TITLE:   LHOI Activity Coordinator
DEPARTMENT:  Adult Day Service
DIVISION: Lincoln Heights Outreach Inc.
REPORTS TO:     ADS Manager

Position Description 

The purpose of this position is to perform professional, friendly, engaging and  stimulating activities for our senior citizens. Administrative work includes, but is not limited to, assessing the unmet social needs of our senior citizens and promoting specialized activity programming to enhance the lives of our senior population. Must put together a monthly activities calendar including scheduling entertainers or speakers and coordinating and running varied activities throughout the day-to-day operations of the center. Must have a valid driver’s license.

The Activity Coordinator will encourage participation in the senior center daily meal program and complement varied activities to be conducted before, after or during the meal.  This position works closely with caregivers, family members and the community at large to market  all essential aspects of the Senior Center.  Knowledge of local social and community events is preferred.

Essential Functions:

  • Responsible for planning and promoting specialized activity programs with supervision from the Manager
  • Must have good computer, basic math and scheduling skills
  • Must be patient and have a positive disposition, demonstrate flexibility and possess ability to multitask
  • Maintains and updates statistical data and evaluates efficiency and effectiveness of activity program
  • Communicates with all disciplines including the Manager and Executive Director
  • Encourages participation at the Senior Center to minimize isolation enhancing the quality of life of our seniors
  • Help market the activity programs to attract new participants
  • Help develop and promote special events
  • Prepare monthly activity calendar
  • Knowledge and experience with Microsoft Word, Excel and Outlook, Facebook and Email, Internet Explorer
  • Requires excellent planning, time management and organizational skills
  • Maintains confidential, written records on each participant as well as records activity services needed, and required follow-up as necessary
  • Requires an ability to handle a wide variety of assignments with frequent interruptions and the ability to reprioritize assignments
  • Keeps calm and handles crises with composure and with a professional, positive attitude
  • Excellent customer service skills with a calm, cooperative, and friendly personality
  • Requires the ability to recognize and anticipate the potential needs of others
  • Positive communicator and empathetic listening skills
  • Requires ability to drive participants in Senior Center van to and from activity programs, if necessary


Minimum Qualifications:

  • High School graduate, experience in related field, some knowledge of activity program/occupational therapy
  • College graduate preferred; with concentration/degree related field, with some knowledge of gerontology
  • Must be CPR certified with AED
  • Must consent to a background check
  • Must have current Ohio driver’s license

Tools and Equipment Used
The employee is required to use all office equipment and have good computer skills. Must be CPR certified with an AED defibrillator and have a current Ohio driver’s license.

Physical Demands
The physical demands listed are representative of those that must be met by the employee to successfully perform the essential functions of this job. Negligible physical effort is required to  perform the duties under typical office conditions. While performing the duties of this job, the employee is frequently required to sit, talk, hear, stand and walk.  The employee must occasionally climb stairs and balance, on occasion lift and/or move up to 10 pounds and bend/stoop. Requires manual dexterity in combination with eye-hand coordination for use of general office equipment and must have overall good motor skills when operating a motor vehicle.

Work Environment
The work environment is performed in an office/center with frequent interruptions.  Must be able to multitask and re-prioritize daily schedules to meet the demands of the day. Has daily contact with local senior citizens, their caregivers and family, as well as the general public and department heads, state representatives, vendors and local business owners.
