For seniors 60 years of age and older who need assistance or supervision in a safe, nurturing environment outside of the home.
Caregivers and providers can be challenged with providing care and activities for their parents, short-term and long-term. Join us in our state-of-the-art facility during the day and experience our exceptional service.
Most insurance is accepted, and we also provide monthly and weekly financial terms.

Eligibility for specific services is determined by a case manager during an in-home assessment.
Funding Sources
PASSPORT is a Medicaid program administered through the Council on Aging of Southwestern Ohio (COA). The purpose of the program is to allow seniors 60 years of age and older to receive nursing facility level of care while remaining in their home.
